Aim 1 Overview: Measurement-Based Care

PURPOSE
The purpose of Aim 1 is to study the impact of providing measurement-based care via remote assessments. Our goal is to make data accessible for both clients and clinicians to create a collaborative relationship and personalized treatment plans that result in greater wellness.

STUDY DESIGN
Aim 1 is a chart review study of measures of symptoms, behavior, functioning, and cognition, delivered as standard of care. There are no other research activities for Aim 1.


OUR APPROACH
In Aim 1 of this study, we are using data in a responsive way to create wellness plans as diverse as the individuals we serve. This innovative approach to measurement-based care uses clients’ lived experience as a roadmap for recovery. As part of our standard of care, each client receives personalized treatment based on information they share with their care team. Data related to symptoms, behavior, functioning, and cognition are gathered securely via remote assessments that are completed on a biannual basis. This detailed picture of each client’s health and lifestyle is then translated into an accessible format called a personal progress summary. Clinicians present each client’s personal progress summary document back to them, which facilitates a shared conversation and a collaborative treatment plan. By building on areas of strength and identifying opportunities for growth, clients can feel confident in the steps they are taking towards wellness.
